Before a facility can submit data to an NRDR registry, a completed Participation Agreement and Business Associate Agreement (BAA) must be in place for the corporate account.
Important: Both agreements must be completed before your facility can begin submitting registry data.

Participation Agreement
The Participation Agreement outlines the responsibilities of both the NRDR and the participating organization, including requirements related to the protection of health information and registry participation.
To complete the Participation Agreement:
- Review the agreement during the NRDR application process.
- Select the acknowledgment checkbox at the bottom of the agreement.
- Click Next to continue.
- Use the Print option if you would like a copy for your records.

Business Associate Agreement (BAA)
The BAA must be signed and submitted to ACR to complete the registration process.
Submit a New BAA
- Download the BAA from the application page.
- Complete all required information.
- Sign the agreement.
- Submit the completed BAA to ACR.

Already Have a BAA on File?
An existing BAA may be reused if:
- It was signed on or after January 1, 2018.
- It was executed within the past three years.
- Any new facility IDs have been added before resubmission.
Important Restrictions
Please Note:
- ACR cannot accept revisions or modifications to the BAA.
- ACR does not review or execute third-party BAAs.
