Invoice - Application Process
Step 1: Enter Facility & Physician Counts
On the Overall Invoice Information page, enter the number of participating facilities and physicians.
Your participation fees are calculated based on these counts.
Step 2: Apply Discount Coupons
If you have a coupon code, enter it and select the applicable facility.
- Multiple coupons may be added.
- Some coupons require a physician NPI number.
Step 3: Choose a Fee Option
| Option | Description |
|---|---|
| Full Registry Access | Fees are calculated for all participating facilities and physicians. |
| Limited Single Database Access | Fees are calculated separately by registry. |
Important: If any facility participates in DIR or GRID, Full Registry Access is required.
Step 4: Review Your Draft Invoice
You can download or print a draft invoice before submitting.
The invoice includes:
- Facility and physician totals
- Applied discounts
- Registry participation fees
- Estimated charges
Step 5: Review the Registration Report
Verify that all information is accurate before submission.
The report includes:
- Corporate Account information
- Facility information
- Dictionary records
- Participation agreements
- Coupon details
- Site and physician counts
- Fee summary
Step 6: Submit Your Application
After reviewing all information:
- Click Submit.
- Your application is sent to ACR for processing.
- The final invoice is emailed to the Corporate Account Administrator.
Payment Options
- Online Payment: Select Continue to proceed to PayPal.
- Check Payment: Follow the instructions displayed on the Online Payment page.
Note: Payment instructions become available after the application is submitted.
