All GRID data may be submitted via data file upload. However, most facilities submit the annual Facility, Monthly Data by Facility, and Monthly Data by Physician forms manually. Exam-level data for turnaround time must be submitted via either a data file upload or web-based data transmission.
Note: You may upload data for multiple facilities simultaneously; however, if you leave the Facility ID blank for a row, that row will be uploaded to the Facility ID selected in the facility picker.
To start the upload process, download one of the templates shown in GRID Upload Templates. The templates are also available for download from the NRDR portal as follows:
- Click Data in the top menu of any page in the NRDR portal.
- Click the drop-down arrow next to GRID in the left menu.
- Click Upload Data File.
- Click the link for the desired version. If you are uploading exam-level data, we highly recommend using GRID Data Version 2.2. Version 2.1 has a number of obsolete fields and will eventually be discontinued.

Enter your data in the template following the file specifications below.
Note: Columns with time values must be formatted as text.

Note: Please ensure that your file does NOT have any formulas in the cells. This will cause the file to reject and potentially not allow the log file to be accessed.
Make sure you name your file according to the following naming conventions. Note the free text field is required and has a 50 character limit. Do not include spaces in the file name.

There are two options for uploading data files:
Use the Upload Data File function on the GRID page of the NRDR portal. See the Data File Upload Instructions article for details.
Use the NRDR Uploader app. The NRDR Uploader app is available for uploading exam-level (turnaround time) data only. It makes the data upload process faster and easier than the Upload Data File function. The main benefits of the app are:
a. Data files process within minutes - you do not need to wait 24 hours for files to process, as when using the Upload Data File function.
b. You can download your log file as soon as processing is finished, allowing you to review rejected records and fix them right away.
